DISABILITY SUPPORT SERVICES

Comprehensive community and individual supports, from life skills and apartment searching to benefits assistance, respite, and guardianship guidance.

Individual Supports

With our Individual Supports program, family members can be hired to care for their loved ones with rates ranging from $20 – $25/hr. This helps ease the stress of finding outside care, so families don’t have to leave their loved one with someone unfamiliar. Instead, support comes from the people who know them best. Support Brokerage

Healthy Moves offers support brokerage services to help individuals and families navigate disability programs and funding options. We provide guidance in planning, budgeting, and connecting with resources so individuals can make informed choices and maintain control over their care. We work alongside you to help you self-direct your services. Here are a few examples of what support brokerage services have been used to accomplish:

  • Finding a new home – Apartment searching, independent living facilities and group home searches tailored to the needs of the individual.
  • Navigate benefits – Assistance with Medicaid applications, social security applications, as well as applying for SNAP, WIC, and Emergency Assistance Funding.
  • Access essential resources – Support in obtaining adaptive equipment, utility assistance, and petitioning for emotional support animals.
  • Community mapping – Identifying local resources, programs, and supports.
  • Enhance your living environment – Petitioning for grants or funding for equipment and home modifications.

Natural Supports

Healthy Moves encourages the use of natural supports—relationships and community connections that foster independence and belonging. When additional expertise is needed, our qualified medical staff are available to provide guidance and assistance. This balanced approach ensures individuals receive both community-based encouragement and professional support when appropriate.

Life Skills

We provide hands-on training in essential life skills to help individuals live more independently. From cooking and cleaning to budgeting and paying bills, to personal care and creating task lists, Healthy Moves teaches practical skills that build confidence and self-sufficiency. Our staff works closely with each person to develop personalized plans that fit their goals and daily routines. For example, an individual might learn how to prepare a simple meal, manage their monthly expenses, or develop a daily hygiene routine with our staff’s support.

career planning

Healthy Moves supports individuals in exploring career interests, developing job skills, and preparing for meaningful employment. We assist with resume building, interview preparation, and on-the-job training opportunities. By fostering independence and confidence, our career planning services help individuals pursue rewarding and sustainable career paths. For example, a participant may practice interview skills with our staff, explore internships in their field of interest, or receive coaching to maintain success in their first job.

RESPIte

Respite services provide temporary relief for families and caregivers, giving them time to rest and recharge while ensuring their loved ones continue receiving safe, quality care. Healthy Moves offers flexible respite options tailored to individual needs, whether at home or in the community. Our goal is to support both individuals and caregivers by promoting balance and peace of mind.

Assistive Technology

Assistive technology can open doors to independence and accessibility. Healthy Moves provides guidance and training on a range of devices and tools that support communication, mobility, and daily living. We ensure individuals and families understand how to use technology effectively to enhance quality of life. Examples include setting up voice-to-text software for school or work, teaching the use of adaptive switches for mobility equipment, or showing families how to use communication devices to enhance daily interactions.

transportation

Reliable transportation is essential for community access and independence. As an approved DDD transportation provider, Healthy Moves offers safe, dependable transportation services for medical appointments, work, and community activities. Our trained staff ensures riders are supported and comfortable every step of the way.

Community Inclusion

Healthy Moves promotes opportunities for individuals to participate fully in community life. Through social, recreational, and volunteer activities, we help individuals build friendships, explore interests, and feel connected. Our programs encourage inclusion, belonging, and meaningful engagement.

Community-Based Supports

We offer flexible supports in real-world settings, helping individuals achieve personal goals within their communities. Whether it’s grocery shopping, attending classes, or managing daily routines, Healthy Moves provides the guidance and encouragement needed to succeed. These services are tailored to foster independence and active participation. For example, staff may accompany someone to the library to practice reading skills, support them in grocery shopping to follow a healthy meal plan, or guide them through community fitness activities.
